Friuli Venezia Giulia Region has been experimenting and supporting for several years strategic policies aimed at strengthening community-based care through:
- Integration of policies and resources from health sector and social inclusion sector
- Support to community development and community-based welfare
- Definition of innovative public-private organisational models
In the attached template you can have an overview of the activities of Friuli Venezia Giulia Region related to the transition from institutional care to community-based alternatives in respect of the elderly and disabled persons (including persons with mental health problems).
